Almost always the content. A site cannot be finished without the words and pictures, and those usually have to come from the business. Slow decisions on changes are the second cause. If you have your text, photos and logo ready on day one, the build goes much faster.

Hosting and care are included for the first 12 months: updates, backups, security and small content changes. After that it is £99.99 a year plus VAT. Larger changes are £55 an hour.
